We accept Anthem Blue Cross Blue Shield.
Anthem Blue Cross Blue Shield can be difficult for us to access quickly, partly because Anthem often subcontracts its vision coverage to other vision insurance companies. We may have to call you back if you have questions about your BCBS coverage. If you have Anthem Blue Cross Blue Shield we can often access it using the primary's ID number, but having the SSN of the primary can help us find your insurance more quickly.
We accept Colorado Access and CHP+.
CHP+ often covers an exam and glasses for its patients.
We can verify Colorado Access or CHP+ by using the state ID number of the patient, the Colorado Access number of the patient, or sometimes with the patients name and date of birth.
We accept Colorado Medicaid. Medicaid in Colorado covers medical office visits and eye exams for all of its patients, and glasses for patients under twenty-one years of age.
To access your Colorado Medicaid we need your name, date of birth, and your State ID as written on your medicaid card. Your state ID will be one letter followed by six numbers.
We accept Colorado Medicare. Medicare in Colorado covers vision exams and medical office visits for its patients. There is often a $20 to $25 copay through Medicare for an exam that we usually bill after Medicare processes the claim and sends us an explanation of benefits (EOB).
If you have not met your deductible for the year then Medicare may apply the cost of the exam to your deductible.
We accept Davis Vision also known as Versant at Eagle Eyes North and have a selection of Davis Vision covered frames there as well as other designer and budget frames that Davis will contribute towards. We do not accept Davis Vision at Citadel Vision Center or Eagle Eyes South.
Different Davis plans have different benefits. Some Davis plans are exam only, some cover an exam and glasses or contacts, and some do not cover services but instead offer a discount.
If you have Davis Vision we can access it using the primary's SSN or Davis Vision ID.
We accept Humana at all three of our locations.
To verify your insurance we will need either your ID number or SS number.
We accept Eyemed Vision Care at all of our offices. There are a wide variety of Eyemed plans. We can explain your benefits to you once we have accessed your insurance.
We can often verify Eyemed with the patients name and date of birth. If you have an ID number for your Eyemed we can also use that. We usually do not need the SSN of the primary or of the dependent but if we are having trouble verifying your insurance we can use a SSN to quickly access your plan.
We accept Spectera vision insurance at all of our offices. The United Healthcare Medicare supplement often contracts its vision services to Spectera.
Optum/Spectera can be verified with the name and date of birth of the primary, with the primary's SSN, or with the subscriber ID number.
We accept TRICARE PRIME, TRICARE FOR LIFE and TRICARE SELECT as eligible insurances. We also offer a 10% OFF military discount for glasses.
We can't bill Tricare for care for Active Duty service members, but Active Duty family members are covered for one exam every year, and retired members with Tricare Prime and their family members are covered for an eye exam every two years.
We need the name, date of birth, and SSN of the primary sponsor and the dependent to verify your Tricare. Tricare does not cover glasses or contacts lenses or contact lens services but does cover vision exams and eye-related medical office visits.
We can see veterans through Triwest VA. Usually Triwest VA will consult with the veteran on their schedule, schedule an appointment on the veteran's behalf, and then notify the veteran.
We can also assist veterans in ordering their glasses from the VA. With the current program started in February 2021, veterans must choose from a selection of frames provided by the VA. We can take measurements and record patient preferences on lens features and transmit that to the VA prosthetics department, which will then mail the glasses to the veteran.
We accept United Health Care at all three of our locations.
The United Healthcare Medicare supplement often contracts its vision services to Optum/Spectera.
If you do have vision coverage through United Health Care and not through Optum/Spectera we can see you at any of our three offices. We need your ID number or SSN to access your United Health Care benefits.
We accept Vision Service Plan, or VSP. There are a wide variety of VSP plans. We can explain your benefits to you once we access your insurance.
Sometimes VSP issues an ID number for your plan, which we can use to find your benefits. If you do not have an ID number, your VSP can usually be verified with either the primary's SSN or the primary's name, date of birth, and the last four numbers of the primary's SSN.
HSA's/Flex Plans are accepted at all three locations.
Upon request, we provide receipts for your Health Savings Account/ Flex Plan submission.
